Comparison of the Analgesic Effect of Single-Port Thoracoscopic Surgery Using Intercostal Nerve Block with Local Incision Closure for Pulmonary Surgery

Abstract:Objective:To compare the analgesic effects of intercostal nerve block and local incision closure in single-port thoracoscopic pulmonary surgery.Method:A total of 76 patients who underwent single-port thoracoscopic lung surgery in Taicang Hospital of Traditional Chinese Medicine from January 2023 to December 2024 were selected and divided into Group A and Group B by random number table method,with 38 cases in each group.Group A was treated with intercostal nerve block for analgesia,while the group B was treated with local incision block for analgesia.The Visual Analogue Scale(VAS)for pain,Ramsay Sedation Scale(RSS),the use of analgesic drugs and the occurrence of complications of the two groups of patients at different time points after surgery were monitored and recorded.Result:At 6 hours,12 hours,24 hours,and 48 hours after the operation,in the resting state and cough state,the VAS of group A was significantly lower than that of group B,and the difference was statistically significant differences(P<0.05).In addition,12 and 24 hours after the operation,the RSS of group A was significantly lower than that of group B,and the difference was statistically significant(P<0.05).The first compression time of PCIA after surgery in group A was significantly later than that in group B.The number of effective compressions and the number of remedial analgesia at 24 hours and 48 hours after surgery were significantly less than those in group B,and the dosage of analgesic drugs at 48 hours was significantly lower than that in group B(P<0.05).The incidences of dizziness and headache(7.89%),urinary retention(2.63%),atelectasis(5.26%),and arrhythmia(2.63%)in group A after the operation were slightly higher than those in group B.There was no statistically significant difference between the two groups(P>0.05).Conclusion:In single-port thoracoscopic pulmonary surgery,the analgesic effect of intercostal nerve block is superior to that of local incision closure.It can more effectively alleviate postoperative pain in patients,reduce the use of analgesic drugs,and does not increase the incidence of complications.
