Effect of Tangshenkang Prescription Combined with Allisartan Isoproxil in Patients with Diabetic Kidney Disease

Abstract:Objective:To observe the effect of Tangshenkang Formula combined with Allisartan Isoproxil axetil in the treatment of diabetic kidney disease(DKD),and to explore its influence on the vascular endothelial nitric oxide synthase(eNOS)-nitric oxide(NO)signaling pathway and renal hemodynamics.Method:100 patients with DKD who were hospitalized or outpatients at Guangdong Clifford Hospital from January to November 2023 were randomly divided into two groups:50 cases in the control group were treated with arrisartan axetil,and 50 cases in the observation group were treated with Tangshenkang prescription combined with Allisartan Isoproxil.The therapeutic effects of traditional Chinese medicine symptoms and the comprehensive therapeutic effects of the disease were evaluated after 6 months.Blood glucose,blood pressure,renal function indicators,the levels of blood eNOS,NO,and vascular endothelial growth factor(VEGF)were detected before and after treatment.The end-diastolic velocity(EDV),peak systolic velocity(PSV),and resistance index(RI)of the renal aorta were also monitored.Result:There was no statistically significant difference in blood glucose and blood pressure between the two groups after treatment(P>0.05).However,the serum creatinine(Scr),blood urea nitrogen(BUN),24-hour protein(UTP),and urine microprotein/creatinine ratio(UACR)in the observation group were lower than those in the control group,while the estimated glomerular filtration rate(eGFR)was higher than that in the control group.The difference was statistically significant(P<0.05).After treatment,the blood eNOS and NO in the observation group were higher than those in the control group,while VEGF was lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the EDV and PSV of the renal aorta in the observation group were higher than those in the control group,while the RI was lower than that in the control group,and the difference was statistically significant(P<0.05).The total effective rates of the therapeutic effects of traditional Chinese medicine symptoms and the comprehensive clinical efficacy in the observation group were both higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The combination of Tangshenkang prescription and arisartan axetil in the treatment of DKD can improve clinical symptoms,enhance clinical efficacy,protect renal function and reduce proteinuria.The mechanism may be related to the regulation of the eNOS-NO signaling pathway and renal hemodynamics.
