The Role of the Ratio of Urea Nitrogen to Serum Albumin Combined with Multiple Indicators in Evaluating the Severity of Acute Pancreatitis

Abstract:Objective:To explore the evaluation value of the ratio of urea nitrogen to serum albumin combined with multiple indicators for the severity of acute pancreatitis(AP)in patients.Method:A retrospective analysis was conducted on 180 patients with AP treated in the Emergency Surgery Department of the First Affiliated Hospital of Bengbu Medical University from January 2022 to May 2025.The patients were divided into a non-SAP group(112 cases)and a SAP group(68 cases).The clinical baseline data of the two groups at admission were analyzed,and binary logistic regression analysis was used to investigate the influencing factors of SAP occurrence.Additionally,ROC curve analysis was conducted to evaluate the predictive value of multiple indicators for SAP occurrence.Result:The age,white blood cell count(WBC),C-reactive protein(CRP),ratio of blood urea nitrogen to albumin(BAR),procalcitonin(PCT),blood urea nitrogen(BUN),Acute Physiology and Chronic Health Evaluation(APACHE Ⅱ)score,and sequential organ failure assessment(SOFA)score of the non SAP group were lower than those of the SAP group,and the differences were statistically significant(P<0.01),while the serum albumin(ALB)of the non SAP group was higher than that of the SAP group,and the difference was statistically significant(P<0.01).logistic regression analysis showed that age,WBC,BUN,BAR,CRP,PCT,APACHE Ⅱ,and SOFA scores were risk factors for SAP occurrence,while ALB was a protective factor for SAP occurrence(P<0.05).ROC curve analysis showed that the age,WBC,BUN,ALB,BAR,CRP,PCT,APACHE Ⅱ score,SOFA score,and comprehensive indicator AUC were 0.741,0.670,0.795,0.157,0.664,0.722,0.774,0.728,0.760,and 0.983,respectively.The sensitivities were 77.9%,75.0%,61.8%,92.9%,66.2%,57.4%,44.1%,77.9%,83.8%,and 98.5%,respectively.And specificities were 61.6%,42.0%,91.1%,64.2%,55.5%,79.5%,87.5%,62.5%,67.9%,and 89.3%,respectively.The cutoff value of the combined index was 0.260,and the total prediction probability was 98.5%.Conclusion:The BAR value has significant value in predicting the occurrence of SAP,and combining multiple indicators has better predictive application value for predicting SAP occurrence.
