Comparison on the Effects of Different Volume of Lavage Fluid in Bronchoalveolar Lavage in Patients with Severe Pneumonia

Abstract:Objective:To explore the effects of different volume of lavage fluid in bronchoalveolar lavage(BAL)on detection rates of pathogenic microorganisms and curative effect in patients with severe pneumonia.Method:A total of 96 patients with severe pneumonia undergoing BAL in Zhangpu Tianfu Hospital were enrolled for retrospective study from February 2024 to February 2025.According to different perfusion volume,they were divided into low perfusion group(60 mL alveolar lavage fluid,50 cases)and high perfusion group(120 mL alveolar lavage fluid,46 cases).The detection results of pathogenic microorganisms in all patients were analyzed.The positive rates of etiological detection in the two groups were statistically analyzed.The curative effect and arterial blood gas analysis indexes before and after treatment were compared between the two groups.Result:There were 102 strains of pathogens from 96 patients,which were mainly on Gram-negative bacteria[78.43%(80 strains)],Gram-positive bacteria[14.71%(15 strains)]and fungi[6.86%(7 strains)].The positive rate of etiological detection in low perfusion group was higher than that in high perfusion group,and the difference was statistically significant(P<0.05).Compared with low perfusion group,response rate of treatment and arterial blood gas analysis indexes were better in high perfusion group,and the difference was statistically significant(P<0.05).Conclusion:The detection rate of pathogenic microorganisms in severe pneumonia varies with the volume of lavage fluid used in BAL treatment.Among them,the detection rate is higher in the low-perfusion group,but the high-perfusion group is more conducive to improving the blood gas indicators of patients.It has better therapeutic effects.
