Application of Blunt Dissection Technique in PICC Insertion for Patients with Hematological Malignancies

Abstract:Objective:To investigate the effect of applying the blunt dissection technique during peripherally inserted central catheter(PICC)insertion in patients with hematological malignancies.Method:From January to December 2024,a total of 87 patients with hematological malignancies who underwent PICC insertion admitted to the Guangdong Second People's Hospital Affiliated to Jinan University were selected and randomly divided into two groups using a random number table method.The control group(n=43)received conventional PICC insertion,while the experimental group(n=44)underwent PICC insertion using the blunt dissection technique.The one-time sheath insertion success rate,pain perception at the puncture site,post-insertion bleeding,number of maintenance procedures within 7 days,and incidence of complications were compared between the two groups.Result:The experimental group demonstrated a notably superior first-attempt sheath insertion success rate compared to the control group(P<0.05).Pain scores at 1 day and 3 days post-procedure were lower in the experimental group than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the pain sensation at the catheterization site 5 days after catheterization(P>0.05).The proportion of a small amount of bleeding within 24 hours after catheterization in the experimental group was significantly lower than that in the control group,and the maintenance frequency within 7 days was significantly less than that in the control group.The differences were statistically significant(P<0.05).The incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of the blunt dissection technique in PICC insertion for patients with hematological malignancies can significantly improve sheath insertion success rate,reduce pain perception,minimize bleeding risk,lower the frequency of maintenance procedures,and enhance the safety of catheter placement.
