Effect of Bilateral Transcutaneous Electrical Acupoint Stimulation at Neiguan(PC6),Shenmen(HT7),and Hegu(LI4)on the Dosage of Analgesics after PFNA Internal Fixation

Abstract:Objective:To analyze the effect of bilateral transcutaneous electrical acupoint stimulation(TEAS)at Neiguan,Shenmen and Hegu on the dosage of analgesic drugs after proximal femoral nail antirotation(PFNA)internal fixation.Method:80 patients who underwent PFNA internal fixation at Liyang Hospital of Chinese Medicine from January 2022 to January 2025 were enrolled.Based on random number table method,they were divided into a control group(n=40)and an observation group(n=40).Both groups were given combined spinal-epidural anesthesia.In the observation group,TEAS was performed 30 minutes before anesthesia induction and 1 to 3 days after the operation.The acupoints were bilateral Neiguan,Shenmen,and Hegu,once a day,for 30 minutes each time.The control group applied electrode patches at the same time points and acupoints,but no electrical stimulation was given.Both groups received patient-controlled intravenous analgesia(PCIA)after the operation.The dosage of sufentanil and the first compression time of the analgesic pump within 48 hours after surgery were compared between the two groups of patients.The Visual Analogue Scale(VAS)scores of pain at rest and during activity at 2,4,8,12,24 and 48 hours after surgery were recorded,and the occurrence of postoperative adverse reactions was statistically analyzed.Result:The dosage of sufentanil within 48 hours after the operation was lower and the first compression time of the analgesic pump was longer in the observation group than in the control group(P<0.05).No significant difference in resting VAS score was found between the two groups at 2,4,8,and 12 hours postoperatively(P>0.05).At 24 and 48 hours postoperatively,the observation group had lower resting VAS scores compared with the control group,with statistically significant differences(P<0.05).At 2,4,8,and 12 hours postoperatively,the observation group had lower VAS scores in motion state compared with the control group,with statistically significant differences(P<0.05).The total incidence of adverse reactions was lower in the observation group,with a statistically significant difference(P<0.05).Conclusion:Bilateral TEAS at Neiguan,Shenmen and Hegu can reduce the dosage of analgesic drugs and pain after PFNA internal fixation surgery,and lower the incidence of postoperative adverse reactions.
