The Effectiveness of the Supervisor's Incision and Drainage Treatment Using a Hanging Wire Branch Tube for Elderly Patients with High-Level Complex Anal Fistulas

Abstract:Objective:To investigate the efficacy of the anastomotic drainage technique involving the incision and suspension of branch tubes in the treatment of complex high anal fistula(CAF)in elderly patients.Method:A total of 122 elderly patients with high-grade anal canal adenocarcinoma admitted to Jingzhou Third People's Hospital between June 2021 and June 2024 were enrolled as study subjects.They were randomly divided into two groups using a random number table,with 61 patients in each group.The control group underwent open suture suspension surgery,while the observation group underwent main canal incision with suture suspension and branch canal anastomotic drainage.Surgical outcomes,anorectal motility parameters,anal function,and complications were compared between the two groups.Result:The hospitalization time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).In the observation group,the postoperative resting pressure of the anal canal,the maximum systolic pressure of the anal canal,the maximum tolerance volume of the rectum,and the rectal sensory threshold were lower than those of the control group,and the differences were statistically significant(P<0.05).The postoperative gas incontinence score of the observation group,fluid incontinence score,solid incontinence score,sanitary pad usage score,and lifestyle change score were lower than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The use of a suture-guided drainage tube for high complex anal fistula in elderly patients yields superior outcomes,shortens wound healing time,accelerates recovery of anorectal motility and anal function,and results in fewer complications.
