Observation on the Efficacy and Recurrence Rate of Combined Lacrimal Drainage Tube Placement and Endoscopic Dacryocystorhinostomy in Patients with Chronic Dacryocystitis

Abstract:Objective:To observe the efficacy and recurrence rate of combined nasolacrimal duct drainage tube placement and endoscopic dacryocystorhinostomy(EDCR)in treating patients with chronic dacryocystitis.Method:102 patients with chronic dacryocystitis treated at Suzhou Lixiang Eye Hospital between January 2023 and June 2024 were retrospectively selected as study subjects,divided into Group A and Group B based on different treatment regimens.Group A(n=54)underwent endoscopic dacryocystorhinostomy(EDCR),while Group B(n=48)received combined treatment with tear duct drainage tube placement and EDCR.Clinical data from two groups were collected,and clinical efficacy was compared between the two groups,including anastomotic healing,granulation tissue formation,visual recovery,lacrimal gland secretion function,tear film breakup time,epiphora severity score,complication incidence,and recurrence rate.Result:Three months postoperatively,epithelialization at the anastomotic site was higher in Group B than in Group A,with a statistically significant difference(P<0.05).The rate of granulation tissue formation was lower in Group B than in Group A,with a statistically significant difference(P<0.05).At 3 months postoperatively,both groups showed decreased Schirmer I test levels and tear overflow scores compared to preoperative levels,while tear film breakup time increased,and the levels of these indicators in Group B were lower than those in Group A,with all differences being statistically significant(P<0.05).There was no statistically significant difference in visual acuity measurements between the two groups at 3 months postoperatively(P>0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).The recurrence rate at 6 months postoperatively was lower in Group B than in Group A,with a statistically significant difference(P<0.05).The overall clinical response rate was higher in Group B than in Group A,with a statistically significant difference(P<0.05).Conclusion:Patients with chronic dacryocystitis benefit significantly more from combined lacrimal duct drainage tube placement and endoscopic dacryocystorhinostomy(EDCR)than from EDCR alone.This approach effectively promotes postoperative recovery and results in a low recurrence rate.
