Efficacy Analysis of Percutaneous Transhepatic Cholangioscopy in the Treatment of Complex Intrahepatic Choledocholithiasis

Abstract:Objective:To assess the clinical outcomes of cholangioscopy-assisted percutaneous transhepatic approaches in managing complex intrahepatic bile duct stones.Method:104 patients with complex intrahepatic bile duct stones admitted to the First Hospital of Sanming City,Fujian Province,from July 2019 to July 2024 were selected as the study subjects,and were divided into the control group(received traditional open surgery treatmen)and the observation group(percutaneous transhepatic choledochoscopy treatment)using the random number table method,with 52 cases in each group.A comparative assessment was performed between groups regarding perioperative parameters,immunological profiles,hepatic function markers,and postoperative complications.Result:In comparison with the control group,the observation group exhibited shorter durations in terms of operation time,intraoperative bleeding volume,postoperative fasting period,postoperative defecation recovery time,and hospital stay.Meanwhile,the stone clearance rate of the observation group was notably higher(P<0.05).After surgery,the levels of CD3+,CD4+,CD8+,CD4+/CD8+,alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin and direct bilirubin showed lower values in contrast to those recorded in the control group(P<0.05).In terms of the complication rate,the observation group presented a lower figure as opposed to the control group(P<0.05).Conclusion:Percutaneous transhepatic cholangioscopy for the treatment of complex intrahepatic bile duct stones can shorten the operative time,reduce intraoperative bleeding,accelerate postoperative recovery,increase the rate of stone removal,improve immune and liver functions,and reduce the complication rate.
