Application Effect of Simple Arterial Ligation Method for Identifying the Intersegmental Plane in Thoracoscopic Anatomical Segmentectomy for NSCLC Patients

Abstract:Objective:To explore the effect of intraoperative application of simple arterial ligation method for identifying the intersegmental plane in patients with non-small cell lung cancer(NSCLC)undergoing thoracoscopic anatomical segmentectomy.Method:The clinical data of 112 patients with NSCLC who were scheduled to receive thoracoscopic anatomical segmentectomy in The Affiliated Suzhou Hospital of Nanjing Medical University(Suzhou Municipal Hospital)from January 2021 to June 2024 were retrospectively analyzed.According to the different intersegmental plane identification methods used during surgery,the patients were divided intotreatment group(n=46)and control group(n=66).Both groups received thoracoscopic anatomical segmentectomy.During surgery,the patients in the treatment groupwere treated with simple artery ligation method,while the patients in the control group adopted modified inflation-deflation method.The surgical conditions in the two groups were observed.The quality of life[Functional Assessment of Cancer Therapy-Lung(FACT-L)]and self-efficacy[General Self-Efficacy Scale(GSES)]were compared between groups before surgery and at 1 month after surgery.The complications in the two groups were recorded within 1 month after surgery.Result:There was no statistical significance in the duration of intersegmental plane occurrence between groups(P>0.05).The total surgical duration,hospitalization duration and intraoperative blood loss in treatment group were shorter or less than those in control group,with statistical differences(P<0.05).There were no statistical differences in FACT-L score and GSES score between groups before surgery(P>0.05),and the FACT-L score and GSES score in the two groups at 1 month after surgery were enhanced compared with those before surgery,and the scores in treatment group were higher than those in control group,with statistical differences(P<0.05).There was no statistical significance between groups in the total incidence rate of pulmonary air leakage,chylothorax and pulmonary infection within 1 month after surgery(P>0.05).Conclusion:Simple arterial ligation method has a good effect in the intersegmental plane identification of NSCLC patients with thoracoscopic anatomical segmentectomy,and it can improve the surgical efficiency and safety,and it is conducive to the improvements of postoperative quality of life and self-efficacy.
