The Effect of Ultrasound-Guided Quadratus Lumborum Block Versus Intravenous Infusion of Lidocaine on Postoperative Analgesia in Patients Undergoing Colorectal Cancer Radical Resection

Abstract:Objective:To evaluate the effect of ultrasound-guided quadratus lumborum block and intravenous lidocaine on postoperative analgesia in patients undergoing radical resection of colon cancer via abdominal surgery.Method:80 patients who underwent transabdominal radical resection of colon cancer in Nantong Third People's Hospital from September 2021 to June 2023 were selected as the study objects.Patients were randomly included in the quadratus lumborum block group(QL group)or the intravenous lidocaine injection group(IL group),and ultimately 40 patients were included in each group.Patients in the QL group underwent ultrasound-guided quadratus lumborum block.Patients in the IL group were intravenously injected with lidocaine at a dose of 1.5 mg/kg before anesthesia induction,and lidocaine at a dose of 1.5 mg/kg/h was continuously pumped during the operation.The NRS was used to evaluate the pain of patients immediately after leaving the PACU at rest and when coughing,2 hours after surgery,4 hours after surgery,12 hours after surgery,and 24 hours after surgery.The number of effective PCIA compressions and the total number of compressions within 24 hours after the operation,the number of patients using parecoxib sodium,the number of patients using tramadol,and the time of first getting out of bed were recorded were recorded.The incidence of postoperative adverse reactions such as postoperative nausea and vomiting(PONV),skin pruritus,dizziness and drowsiness in patients were recorded.Result:There was no statistically significant difference in NRS at each time point after surgery between the two groups(P>0.05).There was no statistically significant difference in the number of effective PCIA compressions within 24 hours after surgery,the total number of PCIA compressions,the number of users of parecoxib sodium,and the number of users of tramadol between the two groups of patients(P>0.05).The time to get out of bed for the first time in the IL group was significantly shorter than that in the QL group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of PONV,pruritus,dizziness and drowsiness between the two groups of patients(P>0.05).No serious adverse reactions related to nerve block or drug-related adverse reactions were observed in either group of patients.Conclusion:In patients undergoing radical resection of colon cancer via abdominal surgery,intravenous lidocaine can achieve postoperative analgesic effects similar to those of ultrasound-guided quadratus lumbosus block and has relatively high safety.
