Diagnostic Value of Transcranial Doppler Ultrasound and Carotid Ultrasound in the Degree of Carotid Artery Stenosis in Patients with Ischemic Stroke

Abstract:Objective:To investigate the diagnostic value of transcranial Doppler ultrasound(TCCD)and carotid ultrasound(CVUS)in the degree of carotid artery stenosis in ischemic stroke patients.Method:Clinical data of 100 ischemic stroke patients diagnosed in the Fourth People's Hospital of Langfang City from December 2022 to December 2024 were retrospectively analyzed.All patients received TCCD,CVUS and digital subtraction angiography(DSA),and the data of 100 patients were complete.TCCD,CVUS and combined diagnostic results were counted,and DSA results were used as the"gold standard"to compare the efficacy of TCCD,CVUS and combined diagnostic results.Result:Among 100 patients DSA diagnosed stenosis in 80 cases and no stenosis in 20 cases;TCCD diagnosed stenosis in 69 cases and no stenosis in 14 cases.CVUS diagnosed stenosis in 68 cases and no stenosis in 15 cases.Combined diagnosis diagnosed stenosis in 78 cases and no stenosis in 17 cases.TCCD had a diagnostic accuracy of 83.00%,sensitivity of 86.25%,and specificity of 70.00%,CVUS had a diagnostic accuracy of 83.00%,sensitivity of 85.00%,and specificity of 75.00%,and the combined diagnostic accuracy of 95.00%,sensitivity of 97.50%,and specificity of 85.00%.The joint diagnostic accuracy and sensitivity were higher than that of TCCD and CVCS,and the difference was statistically significant(P<0.05).The results of DSA diagnosis showed that there were 26 cases of mild,26 cases of moderate,23 cases of severe,and 5 cases of occlusion,and the detection rate of moderate stenosis in the combined diagnosis was 96.15%,which was higher than that of 69.23%in TCCD and 65.38%in CVUS,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the detection rates of mild,severe,and occlusion among the three examination methods(P>0.05).Conclusion:TCCD and CVUS both have some reference value in diagnosing the degree of carotid artery stenosis in ischemic stroke patients,but the diagnostic efficacy of their combined diagnosis is better.
