Effect of Minimally Invasive Small Skull Winolow Surgery on Hematoma Clearance Rate and Prognosis in Elderly Patients with Hypertensive Intracerebral Hemorrhage Combined with Cerebral Hernia

Abstract:Objective:To analyze the effect of minimally invasive small skull winolow surgery in the treatment of elderly patients with hypertensive intracerebral hemorrhage(HICH)complicated with cerebral hernia.Method:60 elderly patients with HICH complicated with cerebral hernia admitted to Qianxi People's Hospital from September 2021 to September 2024 were selected as the study objects and divided into two groups according to the random number table method,with 30 cases in each group.The control group underwent large bone flap craniotomy for hematoma evacuation,while the observation group underwent small bone window minimally invasive surgery.The surgical conditions,neurological functions,inflammatory responses,prognosis and complications of the two groups were compared.Result:The intraoperative blood loss in the observation group was less than that in the control group,the operation time and hospital stay were shorter than those in the control group,the hematoma clearance rate was higher than that in the control group,the incidence of complications was lower than that in the control group,and the prognosis was better than that in the control group.The differences were statistically significant(P<0.05).The National Institutes of Health Stroke Scale(NIHSS)score,neuron-specific enolase(NSE),S-100β protein and other inflammatory indicators after the operation were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Minimally invasive small bone window surgery has advantages such as mild injury and few complications.It can alleviate the neurological function impairment of elderly patients with HICH combined with brain herniation,control inflammation,and help improve the prognosis of patients.
