Relationship between Cardiac Ultrasound Calcification,Apolipoprotein and Coronary Heart Disease and Prediction of Adverse Cardiovascular Events

Abstract:Objective:To explore the relationship between cardiac ultrasound calcification,apolipoprotein and coronary heart disease and the prediction of adverse cardiovascular events value.Method:60 patients with coronary heart disease who were clinically diagnosed and treated in Hezhou Hospital of Traditional Chinese Medicine from June 2023 to August 2024 were selected as the study group,and 60 healthy subjects were selected as the control group during the same period.Cardiac valve calcification and apolipoprotein A1(ApoA1)and Apolipoprotein B(ApoB)were examined in both groups.The levels of cardiac valve calcification score,ApoB and ApoA1 were compared between the study group and the control group,and the correlation between cardiac valve calcification score and ApoB and ApoA1 was analyzed.Patients in the study group were divided into occurrence group and non-occurrence group according to whether they had a major adverse cardiovascular event(MACE).Receiver operating characteristic curve(ROC)was plotted to evaluate the predictive value of cardiac valve calcification score,ApoB,ApoA1 and their combination in MACE.Result:The cardiac valve calcification score and apoB level in the study group were significantly higher than those in the control group,but the ApoA1 level was lower than that in the control group,and the difference was statistically significant(P<0.05).Cardiac valve calcification score was positively correlated with ApoB(r=0.659,P<0.05)and negatively correlated with ApoA1(r=-0.227,P<0.05).60 patients in the study group were followed up for 6 months,including 6 cases of myocardial infarction,1 case of cardiac death,5 cases of revascularization,and 3 cases of heart failure,with a total incidence of 25.0%.The score of cardiac valve calcification and ApoB level were significantly higher than those in the non-occurrence group,and the ApoA1 level was lower than that in the non-occurrence group(P<0.05).Receiver operating characteristic curve analysis results showed that cardiac valve calcification score(AUC=0.709,95%CI:0.562~0.857),ApoB(AUC=0.771,95%CI:0.639~0.904),ApoA1(AUC=0.788,95%CI:0.642~0.934)and the combination of the three(AUC=0.869,95%CI:0.757~0.982)had significant predictive value for MAEC(AUC>0.7,P<0.05),and the combined AUC of the three was higher than any single index.Conclusion:Cardiac ultrasound calcification,ApoB and ApoA1 are closely related to the occurrence and development of coronary heart disease,and they can be used as effective indicators to predict cardiovascular adverse events in patients with coronary heart disease.
