The Effect of Ultrasound-Guided FICB Combined with Low-Dose Ropivacaine Spinal Anesthesia on Hemodynamics and Postoperative Cognition in Patients with PFNA Internal Fixation

Abstract:Objective:To analyze the effects of ultrasound-guided iliac fascial space block(FICB)combined with low-dose ropivacaine spinal anesthesia on hemodynamics and postoperative cognition in patients with proximal femoral anti rotation intramedullary nail(PFNA)fixation.Method:86 patients treated with PFNA internal fixation admitted to The Fifth Hospital of Xiamen from March 2022 to December 2023 were selected as the study subjects.They were randomly divided into a control group and an experimental group,with 43 patients in each group.The control group received conventional lumbar anesthesia,while the experimental group received ultrasound-guided FICB combined with low-dose ropivacaine lumbar anesthesia.The anesthesia effect,hemodynamic changes,postoperative cognitive ability,and incidence of complications were compared between the two groups of patients.Result:The incidence of anesthesia effect in experimental group was lower than that in control group,and the heart rate(HR)and mean arterial pressure(MAP)at body position were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in HR and MAP before anesthesia,at the beginning and after surgery(P>0.05).The score of Mini-Mental State Examination(MMSE)in experimental group was lower than that in control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in experimental group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion:For patients undergoing PFNA internal fixation surgery,the use of ultrasound-guided FICB combined with low-dose ropivacaine spinal anesthesia has a good anesthetic effect,can maintain stable hemodynamic status,promote postoperative cognitive recovery,and reduce the occurrence of complications.It is worthy of research and promotion.
