Effect of Ultrasound-guided Fascia Iliaca Compartment Block Combined with Dexmedetomidine Nasal Drops on Elderly Patients with Hip Fracture

Abstract:Objective:To explore the effect of ultrasound-guided fascia iliaca compartment block(FICB)combined with Dexmedetomidine nasal drops in elderly patients with hip fracture.Method:A total of 108 elderly patients with hip fracture who underwent hip replacement in Xiangtan First People's Hospital from July 2021 to July 2022 were taken as the research object,and the subjects were randomly divided into two groups.Group B(n=54)received FICB under ultrasound guidance before operation,and group A(n=54)received Dexmedetomidine nasal drops on the basis of group B..The general situation of operation,perioperative hemodynamics,sedation,pain degree and incidence of adverse reactions were compared between group A and group B.Result:There was no significant difference between group A and group B(P>0.05).FICB 1 h(T1),there was no statistical significance in heart rate(HR)and mean arterial pressure(MAP)between the two groups(P>0.05);instant transfer bed(T2),immediately after passing the operating bed(T3),when posing(T4),there were no significant differences in HR,MAP than T1 in group A(P>0.05);HR,MAP in group B were higher than those in T1,and HR,MAP in group A were lower than those in group B,the differences were statistically significant(P<0.05).On the day of surgery,1 d after surgery and 2 d after surgery,Hamilton anxiety scale(HAMD)scores in both groups were decreased compared with 1 d before surgery,and the scores in group A were lower than those in group B,Leeds sleep evaluation questionnaire(LSEQ)scores were increased compared with group B,and the scores in group A were higher than those in group B,the differences were statistically significant(P<0.05).The incidence of postoperative delirium and daytime sleepiness in group A were lower than those in group B,and the differences were statistically significant(P<0.05).Conclusion:Ultrasound-guided FICB combined with Dexmedetomidine nasal drops can maintain the stability of hemodynamics during operation,reduce the emotional impact on patients,relieve postoperative pain,and reduce the incidence of postoperative delirium and daytime sleepiness.
