Effect of Ultrasound-guided Posterior QLB Combined with Parecoxib Sodium Analgesia in Elderly Patients Undergoing Laparoscopic Radical Resection of Colorectal Cancer

Abstract:Objective:To investigate the effect of ultrasound-guided posterior quadratus lumborum block(QLB)combined with Parecoxib Sodium analgesia in elderly patients undergoing laparoscopic radical resection of colorectal cancer.Method:A total of 100 elderly patients who underwent laparoscopic radical resection of colorectal cancer in Xiangtan First People's Hospital from January 2020 to October 2023 were selected as the study objects,and they were divided into traditional group(n=50)and combined group(n=50)according to random number table method,in which the traditional group was given Parecoxib Sodium analgesic treatment.The combined group was treated with ultrasound-guided posterior QLB on the basis of the traditional group,and the extubation time of endotracheal intubation,recovery time,incidence of agitation during recovery,hemodynamics,pain degree and the occurrence of complications were compared between the two groups.Result:The extraction time of endotracheal intubation and recovery time in combined group were shorter than that in traditional group,and the incidence of agitation during recovery was lower than that in traditional group,the differences were statistically significant(P<0.05).Before anesthesia,there were no significant differences in mean arterial pressure(MAP)and heart rate(HR)between the two groups(P>0.05).Compared with before anesthesia,MAP in both groups increased at the end of operation and 2 h after operation,and those in the combined group were lower than the traditional group,HR in both groups decreased,and those in the combined group were higher than those in the traditional group,the differences were statistically significant(P<0.05).MAP in both groups decreased at 6 h,12 h and 24 h after operation,and those in the combined group were lower than those in the traditional group,HR in both groups increased,those in the combined group were higher than those in the traditional group,the differences were statistically significant(P<0.05).There was no significant difference in visual analogue scale(VAS)scores between the two groups before anesthesia and 12 h after surgery(P>0.05).At 12 h,24 h and 48 h after surgery,the VAS scores in both groups were higher than that before surgery,and those in the combined group were lower than that in the traditional group at 24 h and 48 h after surgery,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:For elderly patients undergoing laparoscopic radical resection for colorectal cancer,ultrasound-guided posterior QLB combined with Parecoxib Sodium analgesia can shorten the recovery time and extubation time,maintain hemodynamic stability,and have a better inhibition effect on body pain without increasing the occurrence of complications.
