Clinical Comparison of UBED and PELD Techniques in the Treatment of Lumbar Disc Herniation

Abstract:Objective:To compare the clinical effect of unilateral biportal endoscopic discectomy(UBED)and percutaneous endoscopic lumbar discectomy(PELD)in the treatment of lumbar disc herniation(LDH).Method:A total of 98 patients with lumbar disc herniation admitted to Longkou Hospital of Traditional Chinese Medicine from October 2021 to October 2023 were selected and divided into group A(n=49,UBED)and group B(n=49,PELD)according to the random number table method.The operation related indexes,pain degree and lumbar function were compared between the two groups,and the incidence of postoperative complications was counted.Result:The intraoperative fluoroscopy frequency in group B was more than that in group A,and the intraoperative bleeding volume,surgery time and hospital stay were less than those in group A,the differences were statistically significant(P<0.05).The visual analogue scale(VAS)scores of the lower back and legs at 1 d,1 month,3 months after surgery in both groups decreased compared to before surgery,the differences were statistically significant(P<0.05).But compared the VAS scores of the lower back and legs between the two groups,the differences were not statistically significant(P>0.05).Three months after surgery,Oswestry disability index(ODI)scores in both groups were lower than those before surgery,Japanese Orthopaedic Association(JOA)scores were higher than those before surgery,and the ODI score in group A was lower than that in group B,while the JOA score was higher than that in group B,the differences were statistically significant(P<0.05).There was no statistical significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Both UBED and PELD techniques have certain therapeutic effects and safety in the treatment of LDH,which can alleviate patient pain and improve lumbar spine function.Compared with PELD,UBED treatment patients has better postoperative lumbar spine function recovery,but there is more bleeding during UBED surgery,and the surgery and hospitalization time is longer.
