The Impact of Endoscopic Thyroidectomy Via the Chest and Breast Approach and Traditional Thyroidectomy on Neck Function and Cosmetic Evaluation in Patients with Thyroid Tumors

Abstract：Objective:To explore the differences in the application of traditional surgery and transthoracic endoscopic thyroid surgery in the treatment of thyroid tumor diseases,aiming to improve the clinical efficacy of patients.Method:56 patients with thyroid tumors admitted to Hui'an County Hospital from January 2022 to January 2024 were selected as the research objects,and were randomly divided into the control group and the study group using the random number table method.The control group received traditional open surgery,while the study group received endoscopic thyroid surgery through the chest and breast approach.To evaluate the application effect of the two treatment plans by observing the differences in surgical outcomes,trauma stress-immunity and complications between the two groups.Result:There was no significant statistical difference in the comparison of surgical time between the two groups(P＞0.05).The study group had less intraoperative bleeding,less postoperative drainage volume,shorter postoperative neck recovery time,lower postoperative pain scores on the 1st day,shorter postoperative hospital stay,and higher scores on the Vancouver Scar Scale(VSS)and Patient and Observer Scar Assessment Scale(POSAS)in the 1st month after surgery(P＜0.05).The trauma stress indicators including Cortisol(Cor),C-Reactive Parotein(CRP),Interleukin(IL-6)increased in both groups after 3 days of surgery compared with those before surgery,while CD4+and CD4+/CD8+indicators decreased.However,the trauma stress indicators in the study group were lower than those in the control group,and the immune indicators were higher than those in the control group(P＜0.05).The incidence of complications in the study group was low(P＜0.05).Conclusion:In the treatment of thyroid tumor diseases,the use of endoscopic thyroid surgery through the chest and breast approach has a small impact on the body's trauma stress immune production,satisfactory surgical cosmetic effects,and is conducive to the recovery of neck function in patients.
