Effect of Salmeterol Fluticasone on Negative Conversion Rate and Immune Function of Tuberculosis Bacilli in Patients with Pulmonary Tuberculosis Complicated with Chronic Obstructive Pulmonary Disease

Abstract:Objective:To evaluate the efficacy of salmeterol fluticasone combined therapy in patients with pulmonary tuberculosis and chronic obstructive pulmonary disease.Method:58 patients with pulmonary tuberculosis and chronic obstructive pulmonary disease were selected from the Third People's Hospital of Changzhou City from June 2020 to June 2023.The patients were divided into control group and observation group,with 29 cases in each group.The control group was treated with standard Western medicine,while the observation group was given meterofluticasone inhalation.The pulmonary function indexes and arterial blood gas parameters before and after treatment were compared between the two groups.At the same time,the changes of immune indexes in 6 months before and after treatment were compared between the two groups.In addition,the absorption rate of lesions,the proportion of cavity reduction and the negative conversion rate of sputum smear were analyzed after 6 months of treatment in the two groups.Finally,the occurrence of serious adverse reactions during treatment was recorded in both groups.Result:After 2 months of treatment,FEV1,FVC and PaO2 increased,while the PaCO2 decreased,with statistical significance(P<0.05).The FEV1,FVC and PaO2 indexes of the observation group were better than those of the control group,and the PaCO2 level was lower,and the difference was statistically significant(P<0.05).After 6 months of treatment,the ratios of CD3+,CD4+and CD4+/CD8+increased in both groups,and the differences were statistically significant(P<0.05).6 months before and after treatment,there was no significant difference in CD3+,CD4+and CD4+/CD8+between the two groups(P>0.05).There was no statistical significance in comparsion of lesion absorption,cavity shrinkage and sputum negative conversion rate between the two groups after 6 months(P>0.05).The incidence of serious adverse reactions during treatment was not statistically significant between the two groups(P>0.05).Conclusion:For patients with pulmonary tuberculosis complicated with chronic obstructive pulmonary disease,salmeterol fluticasone can effectively improve short-term symptom control,and does not significantly affect the course of pulmonary tuberculosis treatment.
