Prognostic Value of RDW,NLR and PLR for the Prognosis in Patients with End-stage Renal Disease Undergoing Maintenance Hemodialysis

Abstract:Objective:To investigate the the predictive value of red blood cell distribution width(RDW),neutrophil/lymphocyte(NLR),and platelet/lymphocyte(PLR)for the prognosis of maintenance hemodialysis(MHD)patients with end-stage renal disease(ESRD).Method:A total of 100 ESRD MHD patients admitted to Zhangzhou Zhengxing Hospital from January 2020 to December 2022 were selected as the study objects.General data and laboratory indicators of all patients were collected.All the patients were followed up,and the survival status of the patients was used as the prognostic index.According to the survival status of the patients,they were divided into survival group and death group.The follow-up outcomes of ESRD MHD patients were analyzed.The clinical data of survival group and death group were compared.The risk factors of poor prognosis in ESRD MHD patients was analyzed.The prognostic value of RDW,NLR and PLR on ESRD MHD patients prognosis was analyzed.Result:By the end of follow-up,84 patients survived and 16 died.According to the outcome of patients,they were divided into survival group(n=84)and death group(n=16).The levels of blood calcium(Ca),hypersensitive C-reactive protein(hs-CRP),RDW,PLR and NLR in the death group were higher than those in the survival group,while the levels of hemoglobin(Hb)and albumin(ALB)were lower than those in the survival group,the differences were statistically significant(P<0.05).Through multi-factor logistic regression analysis,intra-arteriovenous fistula,diabetic nephropathy,high level RDW,NLR and PLR were independent risk factors for poor prognosis in ESRD MHD patients(P<0.05).The sensitivity,specificity and area under the curve(AUC)of RDW,NLR and PLR combined to predict the prognosis of ESRD MHD patients were 93.75%,90.48%and 0.889 respectively.Conclusion:The expression levels of RDW,NLR and PLR are high in ESRD MHD patients with poor prognosis,and they have certain predictive value for the prognosis of ESRD MHD patients,and the combined diagnosis has higher predictive value.
