Analysis of Predictive Value of Platelet Parameters and Coagulation Function Indicators for Severe MPP and Its Correlation with Prognosis

Abstract:Objective:To explore the predictive value of platelet parameters and coagulation function indicators for severe Mycoplasma pneumoniae pneumonia(MPP)and its correlation with prognosis.Method:A total of 110 children with MPP admitted to Taishan People's Hospital from June 2022 to May 2023 were selected,and 50 healthy physical examination children admitted to the Child Health Care Department during the same period were selected as the control group.According to the severity of the disease,the MPP children were divided into severe group(52 cases)and mild group(58 cases).The laboratory indicators of three groups were detected.The pediatric critical case score(PCIS)of children with MPP and prognosis in the severe group were evaluated.The laboratory indicators of three groups were compared.The influencing factors of severe MPP were analyzed.The predictive value of platelet count(PLT),D-dimer(D-D)and fibrinogen degradation product(FDP)on severe MPP was analyzed.The correlation between PLT,D-D,FDP and PCIS scores in children with severe MPP was analyzed.The laboratory indexes of good prognosis group and poor prognosis group were compared.The influencing factors of poor prognosis in children with severe MPP was analyzed.Result:The PLT,activated partial thromboplastin time(APTT)in mild group and severe groups were lower than those in control group,platelet distribution width(PDW),PLT/lymphocyte count ratio(PLR),D-D,FDP and fibrinogen(FIB)were higher than those in control group,and PLT and APTT in severe group were lower than those in mild group,PDW,PLR,D-D and FDP were higher than those in mild patients,and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that PLT,D-D and FDP were independent predictors of severe MPP(P<0.05).Receiver operating characteristic(ROC)curve analysis results showed that the area under the curve(AUC)of PLT,D-D,FDP and combined detection to predict severe MMP were 0.794,0.845,0.800 and 0.947 respectively.Correlation analysis showed that PLT was positively correlated with PCIS score(r=0.582,P<0.05),D-D(r=-0.646,P<0.05)and FDP(r=-0.727,P<0.05)were negatively correlated with PCIS scores.The level of PLT in the poor prognosis group was lower than that in the good prognosis group,and D-D,FDP and FIB were higher than those in the good prognosis group,and the differences were statistically significant(P<0.05).The results of multivariate analysis suggested that high D-D and FDP levels were independent risk factors for poor prognosis in children with severe MMP(P<0.05).Conclusion:Patients with MPP generally have abnormal condition in platelet parameters and coagulation function indicators.The combined detection of PLT,D-D,and FDP has high predictive value for severe MPP,among which D-D and FDP are related to the prognosis of severe MPP patients.
