Clinical Study and Safety Analysis of Low-dose Insulin Infusion and Continuous Insulin Pump Treatment in Emergency Treatment of Diabetes Ketoacidosis Patients

Abstract:Objective:To study the clinical comparative value and safety of low-dose insulin infusion and continuous insulin pump treatment in emergency treatment of patients with diabetes ketoacidosis (DKA). Method:80 DKA patients admitted to Qianjiang Central Hospital from January 2022 to August 2023 were selected as the study objects,and were divided into control group and treatment group according to the random table method,with 40 cases in each group. The control group was given low-dose insulin drip treatment,and the treatment group was given continuous insulin pump treatment. The indicators at admission,clinical efficacy,24 h blood glucose value,time of ketone body turning negative,pH recovery time,average insulin dosage,other prognostic indicators,occurrence of adverse reactions,the number and incidence of hypoglycemia reaction and overall adverse reactions of the two groups were observed. Result:There was no statistically significant difference in the random blood glucose,glycated hemoglobin,blood ketone body,and urine ketone body related indicators between the two groups of patients upon admission (P>0.05). The controllable rate of clinical efficacy and improvement rate of the treatment group were significantly increased compared with the control group,the incidence of coma or death was significantly decreased compared with the control group,and the total effective rate of the treatment group was higher than that of the control group,the difference was statistically significant (P<0.05). 24 h blood glucose value,ketone body negative time,average insulin dosage,pH recovery time of treatment group were significantly lower than control group,and blood glucose reduction rate was significantly slower than control group,the difference was statistically significant (P<0.05). The rate of adverse reactions in the treatment group was significantly lower than that in the control group,with statistical significance (P<0.05). Conclusion:Continuous treatment with insulin pump in the emergency treatment of DKA patients can improve the success rate of treatment,reduce mortality,and has significant advantages in blood glucose control,ketone body conversion and insulin dosage.
