Effect of Two-stage Minimally Invasive Percutaneous Nephrolithotomy in Patients with Upper Ureteral Calculi Complicated with Infection and Its Influence on Renal Function and Inflammatory Factors

Abstract:Objective:To explore the efficacy of two-stage minimally invasive percutaneous nephrolithotomy in patients with upper ureteral calculi complicated with infection and its influence on renal function and inflammatory factors.Method:Ninety-eight patients with upper ureteral calculi complicated with infection admitted to the Second Affiliated Hospital of Xiamen Medical College from November 2018 to October 2022 were selected as the study objects.According to the random number table method,they were divided into two groups,49 cases in each group.The control group was treated with ureterolithotomy,and the observation group was treated with two-stage minimally invasive percutaneous nephrolithotomy.The calculus clearance rate,operation time,intraoperative blood loss,length of hospital stay,complication rate,renal function indexes[serum creatinine(Scr),blood urea nitrogen(BUN)and neutrophil gelatinase-associated lipocalin(NGAL)]and inflammatory factors[tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β),interleukin-6(IL-6)and interleukin-8(IL-8)]before and after operation were compared between the two groups.Result:There was no significant difference in stone clearance rate between the two groups(P>0.05).The operation time and hospital stay in the observation group were significantly shorter than those in the control group,the amount of intraoperative blood loss was significantly less than that in the control group,and the incidence of complications was significantly lower than that in the control group,the differences were statistically significant(P<0.05).There were no significant differences in renal function indexes and inflammatory factors between the two groups before operation(P>0.05).At 1 d and 3 d after operation,renal function indexes and inflammatory factors in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Two-stage minimally invasive percutaneous nephrolithotomy is effective in patients with upper ureteral calculi complicated with infection,which can significantly improve renal function and inflammatory factor levels.
