Relationship between Klotho Protein,FGF-23 and Chronic Heart Failure Combined with Frailty

Abstract：Objective:To analyze the relationship between Klotho protein,fibroblast growth factor-23(FGF-23)and chronic heart failure combined with frailty.Method:A total of 200 patients with chronic heart failure admitted to Fujian Provincial Hospital from September 2022 to June 2023 were selected as the study objects.Clinical data of all patients were collected.The patients were divided into chronic heart failure combined with frailty group and simple chronic heart failure group,and the patients with chronic heart failure combined with frailty group were divided into mild frailty group,moderate frailty group and severe frailty group.The clinical data of chronic heart failure combined with frailty group and simple chronic heart failure group were compared.The levels of klotho protein and FGF-23 in mild frailty group,moderate frailty group and severe frailty group were compared.The relationship among klotho protein,FGF-23 levels,nutritional indexes,inflammatory factors and weakness level in patients with chronic heart failure combined with frailty was analyzed.Result:The proportion of grade Ⅱ,klotho protein,albumin and hemoglobin in chronic heart failure combined with frailty group were lower than those in simple chronic heart failure group,and the levels of N-teminal-pro-B-tye natriunetic peptide(NT-proBNP),FGF-23,red blood cell volume distribution width,hypersensitive C-reactive protein and tumor necrosis factor-α were higher than those in simple chronic heart failure group,the differences were statistically significant(P<0.05).The level of klotho protein in severe frailty group and moderate frailty group was lower than that in mild frailty group,and the level of klotho protein in severe frailty group was lower than that in moderate frailty group;the level of FGF-23 in severe frailty group and moderate frailty group was higher than that in mild frailty group,and the level of FGF-23 in severe frailty group was higher than that in moderate frailty group,the differences were statistically significant(P<0.05).Spearman results showed that Klotho protein was negatively correlated with frailty degree(rs=-0.286,P<0.001),while FGF-23 was positively correlated with frailty degree(rs=0.373,P<0.001).Pearson correlation analysis showed that klotho protein was positively correlated with albumin and hemoglobin,but negatively correlated with red blood cell volume distribution width,hypersensitive C-reactive protein and tumor necrosis factor-α,FGF-23 was negatively correlated with albumin and hemoglobin,but positively correlated with red blood cell volume distribution width,hypersensitive C-reactive protein and tumor necrosis factor-α(P<0.05).Conclusion:Serum FGF-23 levels increased and klotho protein levels decreased in patients with chronic heart failure combined with frailty,and the levels of klotho protein and FGF-23 were correlated with nutritional indexes and inflammatory factors in patients with chronic heart failure combined with frailty.The determination of serum klotho protein and FGF-23 levels may be helpful for the evaluation of clinical conditions in patients with chronic heart failure combined with frailty.So it can provide reference for clinical treatment.
