Relationship between Insulin Resistance and Liver Fibrosis Indexes in Patients with Type 2 Diabetes Mellitus Complicated with Chronic Hepatitis B

Abstract:Objective:To investigate the relationship between insulin resistance and liver fibrosis indexes in patients with type 2 diabetes mellitus(T2DM)complicated with chronic hepatitis B(CHB).Method:A total of 1 096 CHB patients admitted to Mengchao Hepatobiliary Hospital of Fujian Medical University from January 2021 to January 2022 were selected.They were divided into T2DM complicated with CHB group(n=66)and simple CHB group(n=1 030).The liver fibrosis indexes[type Ⅲ procollagen aminoterminal peptide(PⅢP),type Ⅳ collagen C-terminal propeptide(CⅣ),laminin(LN),hyaluronic acid(HA)]and blood glucose indexes[glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),fasting insulin(FINS),fasting C-peptide(FCP)]were detected in both groups.Homeostasis model assessment of insulin resistance(HOMA-IR)was used to evaluate the insulin resistance levels of the two groups.The correlation between HOMA-IR and liver fibrosis indexes was analyzed.Result:HOMA-IR,HbA1c,FBG,FINS and FCP in T2DM complicated with CHB group were higher than those in simple CHB group,and the differences were statistically significant(P<0.05).The levels of PⅢP,CⅣ,LN and HA in T2DM complicated with CHB group were higher than those in simple CHB group,and the differences were statistically significant(P<0.05).HOMA-IR in T2DM complicated with CHB patients was positively correlated with PⅢP,CⅣ,LN and HA(P<0.05).Conclusion:The combination of T2DM can increase the level of insulin resistance and aggravate liver fibrosis in CHB patients,and the level of insulin resistance in T2DM complicated with CHB patients is positively correlated with liver fibrosis indexes.
