Clinical Efficacy of Two Minimally Invasive Plate Osteosynthesis for the Treatment of Proximal Humeral Shaft Fractures

Abstract:Objective:To explore two minimally invasive plate osteosynthesis(MIPO)in the treatment of proximal humeral shaft fractures.Method:A total of 82 patients with proximal humeral shaft fractures admitted to Weng'an County People's Hospital from June 2020 to June 2022 were selected and randomly divided into two groups,with 41 patients in each group.The control group received MIPO surgery through the lateral approach of the humerus,while the observation group received spiral MIPO surgery.The surgical conditions,pain scores,shoulder joint function,shoulder joint range of motion and incidence of complications between the two groups were compared.Result:The operation time of observation group was longer than that of control group,and the fracture healing time was earlier than that of control group,the differences were statistically significant(P<0.05).There were no statistically significant differences visual analogue scale(VAS)scores between the two groups on 1 d,3 d and 7 d after surgery(P>0.05).The postoperative Constant shoulder joint scores,American Shoulder and Elbow Surgeon'form(ASES)scores,the University of California at Los Angeles shoulder rating scale(UCLA)scores of the two groups were higher than those before surgery,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the postoperative Constant shoulder joint score,ASES score,and UCLA score between the two groups(P>0.05).The degrees of anterior flexion and lateral rotation of the shoulder joint in both groups after surgery were higher than those before surgery,and the differences were statistically significant(P<0.05).There were no statistically significant differences in degrees of anterior flexion and lateral rotation of the shoulder joint after surgery between the two groups(P>0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Both types of MIPO can achieve good results in the treatment of proximal humeral shaft fractures,which can accelerate the recovery of shoulder joint function,while spiral MIPO has faster fracture healing and fewer postoperative complications.
