Abstract:Objective:To investigate the applying value of multiple b-values diffusion weighted imaging on differentiation of benign and malignant cervical lymph node lesions.Method:55 patients with 23 benign cervical lymph node lesions and 32 malignant ones underwent diffusion-weighted imaging at 3.0 T MRI with different b values of 0,400,600,800,1000,1500 and 2000 s/mm2.All lesions were confirmed by pathology.Result:When b-value≤1000 s/mm2,the SNR of DWI increased as the b-value increased, and when b-value =1000 s/mm2,the SNR reached its climax, when b-value >1000 s/mm2,the SNR of DWI decreased as the b-value increased. With b value of 1000 s/mm2,average ADC value of benign lesion was higher than malignant lesion, among them the differences were statistically significant(P<0.05).Average ADC had an acceptable value on differentiation of benign and malignant cervical lymph node lesions with optimal cutoff point of 1.07×10-3 mm2/s,yielding a sensitivity, specificity and the area under the AUC were 85.6%,82.3% and 0.896,respectively.Conclusion:For good image quality and valid differentiation between malignant and benign lymph nodes, the optimized b value of DWI at 3.0 T for cervical lymph nodes is 1000 s/mm2.
