Clinical Study of Laparoscopic Uterine Artery Occlusion Combined with Focus Debridement for Treating Uterine Scar Pregnancy

Abstract:Objective:To investigate the effect,safety of laparoscopic uterine artery occlusion combined with focus debridement for treating uterine scar pregnancy.Method:116 patients with CSP were collected,according to operation mode,they were divided into uterine curettage group(n=29), embolization+uterine curettage group(n=60),block+debridement group(n=27),uterine curettage group was treated with uterine curettage under B ultrasound monitoring.Embolization+curettage group was first done bilateral uterine artery embolization,and done curettage at one day after operation under B ultrasound monitoring.Blocking+debridement group was first done laparoscopic uterine artery embolization ,then done CSP focus debridement.The operation time,bleeding amount,menses recovery time,hospitalization expenses and hospitalization time and complications were compared among three groups.Result:The operation time of uterine curettage group was shorter than that of embolization+uterine curettage group,or blocking+debridement group(P<0.05),there were no significant differences between focal embolization+curettage group and block+debridement group(P>0.05);the bleeding amount of uterine curettage group was significantly higher than that of embolization+curettage group(P<0.05),that of embolization+curettage group was higher than that of blocking+debridement group(P<0.05);the menses recovery time of uterine curettage group had no significant difference from embolization+curettage group(P>0.05),but which was significantly longer than that of block+debridement group(P<0.05);the total hospitalization cost of embolism+curettage group was the highest,that of curettage group was the lowest(P<0.05);the hospitalization time of blocking+debridement group was the shortest(P<0.05),that of embolization+curettage group was the longest(P<0.05). The complications occurrence rate of blocking+debridement group was significantly lower than that of embolism+curettage group(P<0.05).Conclusion:Laparoscopic uterine artery occlusion combined with resection has less trauma,shorter hospital stay,low hospital costs,fewer complications,it can preserve fertility,it is an effective,reliable treatment regimen in treatment for CSP,it is worthy of clinical application.
