Evaluation of the Effect of an Optimized Emergency Protocol on the Treatment of Adolescent Diabetic Ketoacidosis Complicated with Hyperosmolar Hyperglycemic Coma

Abstract:Objective To explore the optimized emergency protocol for adolescent diabetic ketoacidosis compli-cated with hyperosmolar hyperglycemic coma(DKA-HHC)and its clinical application value.Methods A total of 150 adolescent patients with DKA-HHC admitted to our hospital from January 2023 to January 2025 were selected as the study subjects.They were randomly divided into an observation group and a control group using a random number table meth-od,with 75 patients in each group.The observation group received the optimized emergency protocol,while the control group received conventional emergency treatment.Blood glucose indicators(fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin),blood ketone,plasma osmolality,blood gas analysis indicators(pH,HCO₃⁻)and their recovery time were compared between the two groups.The rescue success rate and incidence of complications were also evaluated.Results Before treatment,there were no statistically significant differences in the observation indicators between the two groups(P>0.05).After treatment,all indicators improved in both groups.The observation group showed lower levels of fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin,blood ketone,and plasma osmolality,and higher levels of pH and HCO₃⁻ compared to the control group,with statistically significant differences(P<0.05).The correction times for hyperglycemia,ketonemia,plasma hyperosmolality,and acidosis were shorter in the obser-vation group than in the control group.The rescue success rate was higher in the observation group(96.00%vs.86.67%,P=0.042),and the total incidence of complications was lower(8.00%vs.20.00%,P=0.034).Conclusion For adolescent patients with DKA-HHC,the optimized emergency protocol can effectively promote the recovery of blood glucose,blood ketone,plasma osmolality,and blood gas indicators,shorten the correction time of metabolic disturbances,improve the rescue success rate,and reduce the risk of complications.
