Impact of Pre-hospital Emergency Care on Clinical Efficacy and Prognosis in Elderly Patients with Hypertensive Crisis

Abstract:Objective To investigate the impact of pre-hospital emergency care on clinical efficacy and prog-nosis in elderly patients with hypertensive crisis.Methods Sixty-eight elderly patients with hypertensive crisis ad-mitted to Nantong Tongzhou District Hospital of Traditional Chinese Medicine from January 2023 to July 2024 were included as retrospective study subjects.Based on the actual emergency treatment method,they were divided into an observation group(receiving standardized pre-hospital emergency care,34 case)and a control group(transported to the hospital by family members and receiving routine in-hospital emergency care,34 case).Core process indicators,short-term efficacy,safety,endpoint outcomes,and long-term outcomes were compared between the two groups.Re-sults Compared with the control group,the observation group had significantly lower blood pressure upon arrival at the emergency room(P<0.05),and significantly shorter time from onset to first antihypertensive medication use and time to achieve target blood pressure(P<0.05).After treatment,systolic and diastolic blood pressures improved sig-nificantly in both groups;at 30 min and 60 min post-treatment,systolic and diastolic blood pressures in the observation group were significantly lower than those in the control group(P<0.001).The total incidence of adverse events in the observation group was significantly lower than that in the control group(5.88%vs.20.58%,P<0.05).The in-hospital mortality rate in the observation group was significantly lower than that in the control group(2.94%vs.23.53%,P<0.05).The hospital stay length(P=0.006)and the modified Rankin Scale score at discharge(P<0.001)were signifi-cantly lower in the observation group compared to the control group.After 3 months of follow-up,the quality of life scores improved significantly in both groups,and the score in the observation group was significantly higher than that in the control group(P<0.001).Conclu-sion For elderly patients with hypertensive crisis,the implementation of standardized pre-hospital emergency care can significantly improve emergency efficiency,achieve rapid and stable blood pressure control,reduce the risk of in-hospital mortality and adverse events,and contribute to improved long-term neurological function and quality of life.
