The Application Value of Percutaneous Endoscopic Gastrostomy in Critically Ill Patients

Abstract:Objective To evaluate the clinical value of percutaneous endoscopic gastrostomy(PEG)in criti-cally ill patients.Methods This retrospective study analyzed the clinical data of 42 critically ill patients admitted to Rongqiang Hospital of Fuqing from January 2023 to December 2024.Based on the mode of enteral nutritional support,patients were assigned to the observation group(PEG,n=21)or the control group(conventional nasogastric feeding,n=21).Nutritional status,complications,and outcomes were compared between groups.Multivariable regres-sion analysis was used to determine the independent effect of PEG on outcomes.Results At admission,nutritional parameters did not differ between the two groups(P>0.05).On days 14 and 28 after the intervention,serum albumin,prealbumin,hemoglobin,and body mass index(BMI)were higher in the PEG group than in the control group(all P<0.05).The incidences of nasal mucosal injury,aspiration pneumonia,sinusitis,tube occlusion,and tube dislodge-ment were lower in the PEG group(P<0.05).The PEG group had shorter hospital length of stay(LOS)and ICU LOS(P<0.05).Multivariable regression showed that treatment allocation(β=-2.105,95%CI:-3.842~-0.368;P=0.019)was an independent predictor of hospital LOS;compared with conventional nasogastric feeding,PEG was associated with a mean reduction of approximately 2.11 days in LOS.The Sequential Organ Failure Assessment(SOFA)score at admission(β=0.921,95%CI:0.234~1.608,P=0.010)was positively associated with hospital LOS.Conclusion For critically ill patients requiring long-term enteral nutrition,PEG,compared with conventional nasogastric feeding,more effectively improves nutritional status,significantly reduces nasopharyngeal and aspiration-related complications,and independently contributes to a shorter hospital stay,demonstrating clear clinical advantages and applicability.
