Impact of an Optimized Emergency Workflow on Short-Term Outcomes in Patients With Brainstem Hemorrhage

Abstract:Objective To evaluate the effects of an optimized emergency care model on resuscitation effi-ciency,clinical outcomes,and complications in patients with brainstem hemorrhage.Methods Using a retro-spective study,a total of 78 patients with brainstem hemorrhage admitted to Xiamen Xinglin Hospital from January 2023 to December 2024 were enrolled and allocated by emergency care model to an observation group(optimized emergency model,n=39)or a control group(conventional emergency model,n=39).The optimized model includ-ed establishment of a dedicated stroke team,prehospital-in-hospital information linkage,activation of a fast-track(green channel),and multidisciplinary collaboration.Emergency care efficiency metrics,neurological function(NIHSS),cognitive function(MoCA),limb motor function(Fugl-Meyer Assessment),activities of daily living(modified Barthel Index),and complication rates were compared between groups.Results Compared with the control group,the observation group had significantly shorter onset-to-door time,time to clinical assessment,door-to-treatment time,and time to hemorrhage control(P<0.001).Post-intervention,NIHSS scores at 1 week and 3 months after treatment were significantly lower in the observation group,while MoCA scores,Fugl-Meyer scores,and modified Barthel Index were significantly higher(P<0.05).In addition,the incidences of complications and all-cause mortality were significantly lower in the observation group than in the control group:pneumonia 15.38%vs.38.46%;stress ulcer bleeding 12.82%vs.28.21%;lower-extremity deep venous thrombosis 2.56%vs.23.08%;urinary tract infection 10.26%vs.28.21%;all-cause mortality 2.56%vs.15.38%(P<0.05).Conclusion In the management of brainstem hemorrhage,implementation of an optimized emergency care model significantly im-proves emergency care efficiency,effectively enhances neurological,cognitive,and motor function as well as activities of daily living,and markedly reduces complication rates and mortality.
