Clinical Efficacy of Vitrectomy Combined with Internal Limiting Membrane Inversion and Tamponade for Highly Myopic Macular Hole Retinal Detachment

Abstract:Objective To evaluate the clinical value of vitrectomy combined with internal limiting mem-brane(ILM)inversion and tamponade in the treatment of highly myopic macular hole retinal detachment(HMHRD),and to provide references for managing complex ocular trauma under disaster or emergency conditions.Meth-ods A total of 120 HMHRD patients admitted to the Department of Ophthalmology at the General Hospital of Southern Theater Command from May 2022 to May 2024 were selected and randomly divided into an observa-tion group(treated with vitrectomy combined with ILM inversion and tamponade,60 case)and a control group(treated with vitrectomy combined with ILM peeling,60 case).Postoperative visual acuity recovery,intraocular pressure(IOP)changes,macular hole closure rate,and retinal reattachment rate were compared between the two groups.Results At 1 week,1 month,3 months,and 6 months postoperatively,the best-corrected visual acuity(BCVA)in the observation group was significantly better than that in the control group(all P<0.05).On the first day after surgery,the IOP in the observation group was slightly higher than that in the control group(P=0.001),but no significant differences were observed at other time points(all P>0.05).The macular hole closure rate(96.67%vs.83.33%,P=0.014)and retinal reattachment rate(100%vs.85%,P=0.002)in the observation group were significant-ly higher than those in the control group.Conclusion Vitrectomy combined with ILM inversion and tamponade significantly improves visual outcomes and anatomical success rates in HMHRD patients.This technique is rela-tively simple to perform and facilitates rapid recovery,making it suitable for the efficient management of complex retinal detachment in disaster relief and emergency medical scenarios.
