Study on the Comprehensive Intervention Effect of Family Empowerment Combined with Kangaroo Mother Care and Breastfeeding in Critically Ill Preterm Infants

Abstract:Objective To analyze the clinical value of a time-sequenced collaborative intervention integrat-ing kangaroo mother care(KMC)and breastfeeding within a family empowerment framework in neonatal intensive care unit(NICU)settings for critically ill preterm infants.Methods A total of 64 critically ill preterm infants ad-mitted to our NICU between December 2022 and December 2024 were enrolled and randomly assigned via random number table to either the observation group(KMC+breastfeeding,n=32)or the control group(routine care+for-mula feeding,n=32).Comparative analyses included growth parameters,feeding outcomes,sleep patterns,neurode-velopmental status(assessed by Neonatal Behavioral Neurological Assessment[NBNA]scores),and mater-nal psychological status(evaluated using Self-Rat-ing Depression Scale[SDS]and Self-Rating Anxiety Scale[SAS]scores).Results At birth,no significant differences were observed between groups in head cir-cumference,body length,or weight(P>0.05).At 30 days postnatal,all growth parameters increased significantly in both groups,with more pronounced improvements in the observation group(P<0.001).NBNA subdomain scores(general responses,active/passive muscle tone,primitive reflexes,behavioral abilities)showed no intergroup dif-ferences at 3 days postnatal(P>0.05),but demonstrated significantly greater improvement in the observation group by 30 days(P<0.05).Compared with controls:The observation group exhibited higher daily milk intake,shorter durations of feeding intolerance,faster time to regain birth weight,and earlier achievement of full enteral feeding(P<0.001);Significantly prolonged daytime/nighttime sleep duration(P<0.05)with reduced nighttime awaken-ings and wake time(P<0.001);Lower incidence of feeding intolerance,bronchopulmonary dysplasia,retinopathy,nosocomial infections,and necrotizing enterocolitis(P<0.05).Maternal SAS/SDS scores decreased significantly post-intervention in both groups(P<0.05),with greater reductions in the observation group(P<0.001).Conclu-sion The time-sequenced KMC-breastfeeding protocol within an NICU family empowerment system effectively promotes growth/neurodevelopment in critically ill preterm infants,improves sleep/feeding quality,reduces compli-cations,and enhances maternal psychological wellbeing.
