Comparative Analysis of Clinical Outcomes Between Single-Incision Laparoscopy and Conventional Open Surgery for Acute Appendicitis

Abstract:Objective To analyze the therapeutic efficacy of single-incision laparoscopy versus convention-al open surgery in the management of acute appendicitis.Methods A total of 100 patients with acute appendici-tis treated at Fuzhou Jin'an District Hospital from October 2021 to October 2024 were randomized using a random number table into two groups.The control group underwent conventional open appendectomy(50 case),and the ob-servation group underwent transumbilical single-incision laparoscopic appendectomy(50 case).Inclusion criteria included a clinical diagnosis of acute appendicitis,absence of perforation,and no severe comorbidities.Perioper-ative outcomes,postoperative levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),and interleu-kin-6(IL-6)at 24 hours,and the incidence of complications were compared between groups.Result Compared with open surgery,the single-incision laparoscopic group had a longer operative time(P<0.05)but demonstrated less intraoperative blood loss,a smaller incision,earlier return of bowel function,shorter time to tube removal,and a shorter postoperative hospital stay(all P<0.05).At 24 hours postoperatively,levels of TNF-α,CRP,and IL-6 were significantly lower in the single-incision laparoscopic group than in the open group(all P<0.05).The over-all complication rate was also lower in the single-incision group(2.00%vs.20.00%,P<0.05).Conclusion For acute appendicitis,transumbilical single-incision laparoscopic appendectomy is a minimally invasive option that is superior to conventional open surgery in reducing surgical trauma,attenuating the systemic inflammatory response,and promoting faster postoperative recovery.
