Precipitating Factors and Treatment of Acute Kidney Injury Complicating Nephrotic Syndrome

Abstract:Objective To investigate precipitating factors of acute kidney injury(AKI)in patients with ne-phrotic syndrome and to explore effective treatment approaches.Methods This study included 36 patients with nephrotic syndrome complicated by AKI who were treated at Zhaoyuan People's Hospital from March 2023 to March 2024.The distribution of precipitating factors and treatment outcomes were analyzed.Results Among the 36 patients,infection was the most common precipitating factor(n=25,69.44%).Hypovolemia-related factors were identified in 5 patients(13.88%),drug-induced nephrotoxicity in 3 patients(8.33%),renal vein thrombosis in 2 pa-tients(5.56%),and active autoimmune disease in 1 patient(2.78%).Following treatment,serum creatinine,blood urea nitrogen,and 24-hour urinary protein excretion significantly decreased(all P<0.001),while creatinine clearance sig-nificantly increased(P<0.001).The overall clinical response rate was 83.33%,and treatment outcomes differed across groups with different precipitating factors(P=0.024).Conclusions Infection is the leading precipitating factor for AKI in patients with nephrotic syndrome.A comprehensive treatment strategy achieved favorable outcomes.
