Application of Routine Coagulation Function Tests Combined with D-Dimer in the Diagnosis of Acute Lower Extremity Deep Vein Thrombosis

Abstract:Objective To investigate the diagnostic value of routine coagulation function tests combined with D-dimer for acute lower extremity deep vein thrombosis(DVT).Methods A total of 80 patients with acute lower extremity DVT admitted to Ninghua County General Hospital from June 2022 to December 2024 were select-ed as the case group.Additionally,60 patients with clinical manifestations of unilateral lower limb swelling,pain,or redness but confirmed by color Doppler ultrasound to exclude DVT were included as the disease control group.Routine coagulation parameters and D-dimer levels were compared between the two groups,and their diagnostic value for acute lower extremity DVT was analyzed.Results Prothrombin time(PT)and activated partial throm-boplastin time(APTT)were significantly shorter in the case group,while fibrinogen(FIB)and D-dimer levels were significantly higher compared to the control group(all P<0.001).Multivariate logistic regression analysis showed that prolonged PT(OR=0.116,95%CI:0.055~0.247)and APTT(OR=0.490,95%CI:0.381~0.630)were protective factors against acute lower extremity DVT,while elevated FIB(OR=13.947,95%CI:4.275~45.496)and D-dimer(OR=1.202,95%CI:1.149~1.257)were independent risk factors.ROC curve analysis demonstrated that the com-bined detection had superior diagnostic efficacy(sensitivity 97.50%,specificity 85.00%,accuracy 91.82%)com-pared to individual indicators.Conclusion Routine coagulation function tests combined with D-dimer detection have significant clinical value in the diagnosis of acute lower extremity DVT.
