Logistic Regression Analysis of Risk Factors for Retinopathy of Prematurity in Preterm Infants with Gestational Age ≤32 Weeks

Abstract:Objective To analyze the incidence and risk factors of retinopathy of prematurity(ROP)in preterm infants with gestational age ≤ 32 weeks,providing evidence for early clinical identification of high-risk infants,optimization of screening strategies,and development of interventions.Methods This retrospective case-control study enrolled 201 preterm infants admitted to the NICU of Xiamen Children's Hospital between Jan-uary 2019 and May 2024.Participants were stratified into ROP and non-ROP groups based on fundus screening results.Potential risk factors were analyzed using logistic regression.Results Among 201 preterm infants,40(19.9%)developed ROP.Univariate analysis revealed significant associations between ROP and gestational age,birth weight,1-/5-minute Apgar scores,singleton pregnancy,hospitalization duration,neonatal respiratory distress syndrome(NRDS),neonatal sepsis,bronchopulmonary dysplasia(BPD),oxygen concentration ≥40%,oxygen ther-apy>7 days,blood transfusion,pulmonary hemorrhage,surfactant replacement therapy,delivery mode,and assist-ed reproductive technology.Multivariate logistic regression identified smaller gestational age(OR=0.327,95%CI:0.196~0.545),lower birth weight(OR=0.997,95%CI:0.994~0.999),and BPD(OR=4.006,95%CI:1.121~14.320)as independent risk factors for ROP.Conclusion Preterm infants with smaller gestational age,lower birth weight,and comorbid BPD constitute a high-risk population for ROP development.Targeted interventions and standardized fundus screening should be implemented to mitigate ROP occurrence in these vulnerable neonates.
