Abstract:Objective To evaluate the therapeutic effect of furosemide combined with levosimendan in pa-tients with acute heart failure. Methods A total of 150 patients with acute heart failure admitted to Anxi County Hospital between January 2021 and November 2024 who met predefined inclusion and exclusion criteria were en-rolled.Using computer-generated random numbers, patients were assigned by simple randomization to three groups(n=50 per group),ensuring equal allocation probability. The combination group received furosemide plus levosime-ndan;Control Group 1 received furosemide alone;Control Group 2 received levosimendan alone. The groups were compared with respect to overall therapeutic response rate, cardiac function parameters, serum markers of myocardial injury, inflammatory markers, and clinical outcomes. Results The overall response rate was significantly higher in the combination group(96.00%)than in Control Group 1(82.00%)and Control Group 2(74.00%),P<0.05.After treatment, improvements in cardiac function(LVEF,SV,NT-proBNP),myocardial injury biomarkers(CK,CK-MB,cTnI),and inflammatory markers(CRP,SAA,IL-6)were significantly greater in the combination group than in both control groups(P<0.001).The overall incidence of adverse events was lower in the combination group(8.00%)com-pared with Control Group 1(20.00%)and Control Group 2(18.00%),P<0.001.Clinical outcomes showed that the combination group had the lowest 180-day mortality(4.00%vs.18.00%vs.14.00%),readmission rate, and adverse event rate, and the highest rate of cardiac function improvement(all P<0.001).Conclusion In acute heart failure, combined therapy with furosemide and levosimendan yields superior improvement in cardiac function compared with either agent alone and,by attenuating inflammatory acti-vation,better suppresses the inflammatory response and reduces myocardial injury.
