Clinical Efficacy of Hemoperfusion Combined with Continuous High-Volume Hemofiltration in Sepsis Complicated with Acute Kidney Injury

Abstract:Objective To investigate the clinical efficacy of hemoperfusion(HP)combined with con-tinuous high-volume hemofiltration(CHVHF)in the treatment of sepsis complicated with acute kidney injury(AKI).Methods A total of 41 patients with sepsis and AKI admitted to Gutian County Hospital from Janu-ary 2022 to December 2024 were enrolled and randomly divided into an observation group(HP combined with CHVHF,21 case)and a control group(CHVHF alone,20 case)using a random number table.The following pa-rameters were compared between the two groups:renal function indicators,including blood urea nitrogen(BUN),serum creatinine(SCr),cystatin C(CysC),and 24-hour urine output;inflammatory markers,including C-reactive protein(CRP),white blood cell count(WBC),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6);as well as survival outcomes.Results Before treatment,there were no significant differences in baseline indicators(renal function:BUN,SCr,CysC,24h urine output;inflammatory markers:CRP,WBC,TNF-α,IL-6)between the two groups(all P>0.05).After treatment,all indicators improved in both groups,with significantly greater improvement in the observation group.At 3 and 7 days post-treat-ment,the observation group showed significantly lower levels of renal function indicators(BUN,SCr,CysC)(all P<0.05)and significantly higher 24-hour urine output(P<0.001)compared to the control group.Mean-while,the observation group also exhibited significantly lower levels of inflammatory markers(CRP,WBC,TNF-α,IL-6)(all P<0.05).Furthermore,the observation group had significantly higher in-hospital survival(100.00%vs.80.00%)and 3-month post-discharge survival rates(90.48%vs.60.00%)compared to the control group(all P<0.05).Conclusion In the clinical treatment of sepsis complicated with AKI,hemoperfusion combined with continuous high-volume hemofiltration significantly improves renal function,reduces inflammatory response,and enhances patient survival rates.
