Efficacy Observation of Early Goal-Directed Kidney Replacement Therapy in Severe Pneumonia-Related Acute Kidney Injury

Abstract:Objective To investigate the clinical efficacy of early goal-directed kidney replacement therapy in the treatment of severe pneumonia-related acute kidney injury.Methods A retrospective analysis was con-ducted on the clinical data of 70 patients with severe pneumonia accompanied by acute kidney injury who were ad-mitted to Bayannur Hospital from January 2022 to January 2024.Based on treatment methods,patients were divided into the observation group(early goal-directed kidney replacement therapy,n=35)and the control group(conven-tional therapy,n=35).Renal function indices,immune markers,and inflammatory markers were compared between the two groups.Results Prior to treatment,there were no significant differences between the two groups in renal function,immune,and inflammatory markers:serum creatinine(P=0.389),blood urea nitrogen(P=0.476),uric acid(P=0.371),Toll-like receptor 2(TLR2,P=0.321),Toll-like receptor 4(TLR4,P=0.252),procalcitonin(P=0.257),interleukin-6(IL-6,P=0.452),and C-reactive protein(CRP,P=0.466).After treatment,the observation group showed significantly lower levels of serum creatinine(P=0.001),blood urea nitrogen(P<0.001),uric acid(P<0.001),TLR2(P=0.007),TLR4(P=0.001),procalcitonin(P=0.005),IL-6(P=0.003),and CRP(P=0.001)compared to the control group.Conclusion In patients with severe pneumonia-related acute kidney injury,early goal-directed kidney replacement therapy can effectively improve renal function and immune response.
