Diagnostic Efficacy of Ultrasound in Suspected Acute Abdominal Syndrome and Analysis of Misdiagnosis and Missed Diagnosis

Abstract:Objective To evaluate the diagnostic efficacy of ultrasound in patients with suspected acute abdominal syndrome(AAS),analyzing its sensitivity,specificity,positive predictive value,negative predictive val-ue,and accuracy.Additionally,to investigate the occurrence of missed diagnoses and misdiagnoses in ultrasound assessments for AAS.Methods A retrospective analysis was conducted on the clinical data of 60 patients with suspected acute abdominal syndrome who visited Bijie Traditional Chinese Medicine Hospital from February 2023 to August 2024.All patients underwent ultrasound examinations.The pathological results were used as the gold standard for analyzing the diagnostic efficacy of ultrasound,and the cases of missed and misdiagnosis were further analyzed.Results Among the 60 patients with suspected AAS,56 patients were diagnosed with acute abdomi-nal syndrome(93.33%)and 4 patients were diagnosed with ordinary abdominal pain(6.67%)based on pathological results.Using the surgical pathological results as the gold standard,the sensitivity of ultrasound in diagnosing AAS was 94.64%(53/56),the specificity was 75.00%(3/4),the positive predictive value was 98.15%(53/54),the neg-ative predictive value was 50.00%(3/6),and the accuracy was 93.33%(56/60).Among the 56 patients diagnosed with AAS by pathology,50 patients had ultrasound results consistent with pathological results(89.29%),while 6 patients had discrepancies between ultrasound and pathology(10.71%).Among these discrepancies,4 were mis-diagnoses(one case each of urinary stone obstruction,ectopic pregnancy,acute intestinal obstruction,and gastro-intestinal perforation),and 2 were missed diagnoses(one case of acute appendicitis and one case of acute pancreatitis).Conclusion Ultrasound examination has high sensitivity and accuracy in diagnosing acute abdominal syndrome,and in most cases,it provides consistent diagnostic results.However,there are certain in-stances of misdiagnosis and missed diagnoses,particularly involving conditions such as urinary stone obstruction,ectopic pregnancy,acute intestinal obstruction,gastrointestinal perforation,acute appendicitis,and acute pancrea-titis.Therefore,ultrasound should be combined with other diagnostic methods to further improve the accuracy of diagnosis.
