Relationship Between Serum Hepcidin and Ferritin Ratio and Clinical Treatment Outcomes in Patients with Acute Myocardial Infarction Complicated by Acute Left Heart Failure

Abstract:Objective This study aims to explore the relationship between the serum hepcidin/ferritin ratio and the clinical treatment outcomes in patients with acute myocardial infarction(AMI)complicated by acute left heart fail-ure(ALHF).Methods A retrospective analysis was conducted on the clinical data of 120 patients with AMI com-plicated by ALHF,admitted to Inner Mongolia Aerospace Hospital from December 2023 to December 2024.Based on the clinical efficacy 1 week after treatment,the patients were divided into the effective group(96 patients)and the inef-fective group(24 patients).The serum levels of hepcidin,ferritin,and the serum hepcidin/ferritin ratio were compared between the two groups.The correlation between serum hepcidin,ferritin,and the hepcidin/ferritin ratio at admission and clinical treatment outcomes was analyzed using point-biserial correlation.The predictive value of these indica-tors for clinical treatment outcomes was assessed using receiver operating characteristic(ROC)curve analysis.Re-sults Compared to the ineffective group,the effective group had significantly lower serum hepcidin levels and hepci-din/ferritin ratios at admission and 1 week after treatment(P<0.001),while the serum ferritin levels were significantly higher(P<0.001).In the effective group,serum hepcidin and hepcidin/ferritin ratios significantly decreased after 1 week of treatment(both P<0.00l),and serum ferritin levels significantly increased(P<0.001).In contrast,no significant changes were observed in these indicators in the ineffective group(P>0.05).Point-biserial correlation analysis showed that clinical treatment outcomes were positively correlated with serum hepcidin levels at admission(r=0.331)and hepcidin/ferritin ratio(r=0.458)(both P<0.001),and negatively correlated with serum ferritin levels(r=-0.324)(P<0.001).ROC curve analysis indicated that serum hepcidin,ferritin,and the hepcidin/ferritin ratio at admission were all valuable predictors of clinical treat-ment outcomes(AUCs of 0.731,0.728,and 0.803,respectively;all P<0.001),with the hepcidin/ferritin ratio showing the highest predictive power(AUC=0.803,sensitivity 71.76%,specificity 82.07%).Conclusion The serum hepci-din/ferritin ratio is closely associated with clinical treatment outcomes in patients with AMI complicated by ALHF and can serve as an important predictor of patient treatment efficacy.
