Clinical Value of Abdominal Ultrasound in Hemodynamic Assessment of Portal Hypertension and Portal Vein Tumor Thrombus in Patients with Primary Liver Cancer

Abstract:Objective To analyze the clinical value of abdominal ultrasonography in evaluating portal hy-pertension and hemodynamic changes related to portal vein tumor thrombosis in patients with primary liver can-cer.Methods A total of 60 patients with primary liver cancer who visited Bijie Traditional Chinese Medicine Hospital from January 2022 to December 2024 were selected for this study.The portal vein diameter(Dpv),portal vein blood flow velocity(Vpv),portal vein blood flow volume(Qpv),splenic vein diameter(Dsv),splenic vein blood flow velocity(Vsv),and splenic vein blood flow volume(Qsv)were compared between patients with and without portal hypertension.The correlation between Dpv,Dsv,and the degree of esophagogastric varices was ana-lyzed.The hepatic artery diameter and blood flow velocity were compared between patients with and without portal vein tumor thrombosis.Results Compared to patients without portal hypertension,patients with portal hyper-tension had significantly wider Dpv and Dsv,slower Vpv and Vsv,and lower Qpv and Qsv,all P<0.05.In patients with Dpv<1.3 cm and Dsv<0.9 cm,the degree of esophagogastric varices was mainly mild.For patients with Dpv between 1.3-1.4 cm and Dsv between 0.9-1.0 cm,the varices were mainly moderate,while for patients with Dpv>1.4 cm and Dsv>1.0 cm,the varices were mainly severe.Significant differences in the degree of esophagogastric varices were observed among different Dpv and Dsv groups(P<0.05),and the degree of esophagogastric varices was positively correlated with Dpv and Dsv(P<0.05),as indicated by the trend chi-square test).Among the 60 liver cancer patients,32 had portal vein tumor thrombosis,accounting for 53.33%.Compared to patients without portal vein tumor thrombosis,those with portal vein tumor thrombosis had significantly wider hepatic artery diameter and faster hepatic artery blood flow veloc-ity.Conclusion In patients with primary liver cancer,those with portal hypertension show dilation of the por-tal and splenic veins,slower blood flow velocity,and reduced blood flow volume.The degree of esophagogastric varices is positively correlated with the diameter of the portal and splenic veins.In patients with portal vein tumor thrombosis,the hepatic artery diameter and blood flow velocity are significantly increased.Abdominal ultrasonog-raphy demonstrates high clinical value in assessing portal hypertension and portal vein tumor thrombosis.
