Efficacy of Laparoscopic Transabdominal Preperitoneal Hernia Repair in the Treatment of Incarcerated Inguinal Hernia

Abstract:Objective To investigate the clinical efficacy of laparoscopic transabdominal preperitoneal her-nia repair for the treatment of incarcerated inguinal hernia.Methods A simple random sampling method was employed to select 60 patients diagnosed with incarcerated inguinal hernia at Lianjiang County General Hospital between April 2020 and April 2024.The patients were randomly assigned into two groups using a digital random number table.The control group(n=30)underwent traditional open tension-free hernia repair,while the observa-tion group(n=30)received laparoscopic transabdominal preperitoneal hernia repair.The two groups were compared in terms of operative time,time to first ambulation,length of hospital stay,intraoperative blood loss,time to gastro-intestinal function recovery,incidence of complications,postoperative pain scores,and serum levels of C-reactive protein(CRP),procalcitonin(PCT),and tumor necrosis factor-alpha(TNF-α)measured preoperatively and post-operatively.Results There was no statistically significant difference in operative time between the two groups(P>0.05).The observation group demonstrated a shorter length of stay,earlier ambulation,and quicker recovery of gastrointestinal function,with significantly less intraoperative blood loss compared to the control group(P<0.001).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Postoperative pain scores on days 1 and 3 were significantly lower in the observation group than in the control group(P<0.001).Preoperatively,there were no statistically significant differences in CRP,PCT,and TNF-α levels between the two groups(P>0.05);however,postoperative levels of CRP,PCT,and TNF-α increased in both groups compared to preoperative levels,with the obser-vation group showing significantly lower inflammatory marker levels than the control group(P<0.05).Con-clusion Laparoscopic transabdominal preperitoneal hernia repair demonstrates favorable clinical outcomes in managing incarcerated inguinal hernia,significantly reducing hospital stay,intraoperative blood loss,and accelerat-ing gastrointestinal function recovery,while effectively decreasing the incidence of complications and resulting in a milder postoperative inflammatory response.
