Research on the Application of Cryoprecipitate Coagulation Factors Combined with Restrictive Fluid Resuscitation in the Treatment of Traumatic Hemorrhagic Shock

Abstract:Objective This study aims to explore the application value of cryoprecipitate combined with re-strictive fluid resuscitation in the management of patients with traumatic hemorrhagic shock.Methods A retro-spective analysis was conducted on the clinical data of 108 patients with traumatic hemorrhagic shock treated at the People's Hospital of Sanshui District,Foshan City,from November 2023 to November 2024.Patients were divided into an observation group(cryoprecipitate+restrictive fluid resuscitation,n=54)and a control group(restrictive fluid resuscitation alone,n=54),based on whether cryoprecipitate was included in the treatment regimen.Coagula-tion parameters,including prothrombin time(PT),activated partial thromboplastin time(APTT),and thrombin time(TT),as well as hemodynamic indicators,such as mean arterial pressure(MAP),central venous pressure(CVP),heart rate(HR),and hematocrit(HCT),were compared between the two groups.Additionally,the recovery from shock and incidence of complications were assessed.Results Prior to resuscitation,there were no significant differences in coagulation parameters(PT,APTT,TT)or hemodynamic indicators(MAP,CVP,HR,HCT)between the two groups.Post-resuscitation,PT,APTT,TT,and HR significantly decreased,while MAP,CVP,and HCT significantly increased in both groups compared to pre-resuscitation values.Notably,the observation group exhibited significantly lower PT,APTT,TT,and HR compared to the control group(P<0.05),while MAP,CVP,and HCT were significantly higher(P<0.001).After resuscitation,the shock index in the ob-servation group was significantly lower than that in the control group(P<0.001),and the lactate clearance rate was significantly higher(P<0.001).The overall incidence of complications in the observation group was significantly lower than in the control group(8.89%vs.24.44%,P<0.05).Conclusion The combination of cryoprecipitate and restrictive fluid resuscitation effectively improves coagulation function and hemodynamic parameters in patients with traumatic hemorrhagic shock,promotes recovery from shock,and reduces the risk of complications.
