Efficacy Observation of Subacute Surgical Treatment for Intertrochanteric Fractures in Elderly Patients

Abstract:Objective To analyze the efficacy of subacute emergency surgery in elderly patients with inter-trochanteric femoral fractures.Methods A retrospective study was conducted involving 64 elderly patients with intertrochanteric femoral fractures who underwent surgical treatment at Longyan Second Hospital from September 2023 to March 2024.Patients were divided into two groups based on the timing of surgery:the observation group(subacute emergency surgery,performed within 24 to 48 hours post-injury)and the control group(elective surgery,performed more than 48 hours post-injury).perioperative indicators,incidence of complications during hospital-ization,improvement in hip joint function,and activities of daily living(ADL)were compared between the two groups.Results The length of hospital stay in the observation group was significantly shorter than that of the control group(P<0.001).During hospitalization,the rates of electrolyte disturbance,delirium,pulmonary infection,urinary tract infection,pressure sores,and lower extremity venous thrombosis in the observation group were signifi-cantly lower than those in the control group(P<0.05).There were no significant differences between the two groups regarding the number of blood transfusions,upper gastrointestinal bleeding,or cardiovascular events(P>0.05).Pre-operatively,there were no significant differences in hip joint function as assessed by the Harris score and activities of daily living(ADL)score between the two groups(P>0.05).Postoperatively,both groups exhibited an increasing trend in Harris scores and ADL scores at 3,6,and 12 months,with the observation group showing significantly higher Harris scores and ADL scores at 3 and 6 months compared to the control group(P<0.05).However,at 12 months post-surgery,there were no significant dif-ferences in Harris scores and ADL scores between the two groups(P>0.05).Conclusion For elderly pa-tients with intertrochanteric femoral fractures,implementing subacute emergency surgery can shorten hospital stay,reduce the risk of complications during hospitalization,and promote the recovery of hip joint function and activities of daily living.
