Analysis of the Application Value of Abdominal and Vaginal Color Doppler Ultrasound Combined Examination in the Diagnosis of Acute Abdominal Conditions in Obstetrics and Gynecology

Abstract:Objective To investigate the application value of color Doppler ultrasound in the diagnosis of acute abdominal conditions in gynecology and obstetrics,and to compare the diagnostic efficacy of abdominal ultrasound,transvaginal ultrasound,and their combined use in acute abdominal conditions.Methods A retro-spective analysis was conducted on the clinical data of 200 patients with acute abdominal conditions who presented to the Obstetrics and Gynecology Department of the 970th Hospital of the People's Liberation Army at the Weihai campus from July 2022 to July 2024.Hysteroscopic examination results were used as the gold standard to compare the diagnostic efficacy of abdominal ultrasound,transvaginal ultrasound,and their combined use for gynecologi-cal and obstetric acute abdominal conditions.Results For the diagnosis of ectopic pregnancy,both abdominal ultrasound and transvaginal ultrasound,as well as their combination,demonstrated significant diagnostic value,with diagnostic efficacy increasing in order(P<0.001).Specifically,the combination of abdominal and transvagi-nal ultrasound achieved a sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of 100.00%,outperforming either ultrasound method alone.In the diagnosis of ovarian cyst torsion,transvaginal ultrasound exhibited higher diagnostic efficacy than abdominal ultrasound,with the combined approach further en-hancing diagnostic performance(P<0.001).The sensitivity of the combined examination was 100.00%,specificity was 98.68%,and accuracy reached 99.00%.For the diagnosis of luteal rupture,a similar trend to that of ovarian cyst torsion was observed,with transvaginal ultrasound exhibiting superior diagnostic efficacy com-pared to abdominal ultrasound,and the combination of both methods improving diagnostic accuracy(P<0.001).The sensitivity and negative predictive value of the com-bined examination were both 100.00%,while the specificity was 98.72%.Conclusion In the diagnosis of acute abdominal conditions in gynecology and obstetrics,transvaginal ultrasound demonstrates superior diagnostic value compared to abdominal ultrasound.The combination of both modalities further enhances diagnostic efficacy,reduc-ing the risks of missed and misdiagnoses,and allowing for a more accurate assessment of the type of acute abdomi-nal condition.
