Abstract:Objective To investigate the clinical efficacy of cosmetic suturing techniques compared to debridement suturing techniques in patients with facial trauma, and to compare the differences between the two methods in terms of wound healing, scar reconstruction, postoperative pain, and inflammatory response.Meth-ods A retrospective analysis was conducted on the clinical data of 50 patients with facial trauma who presented to the emergency department of Bayannur City Hospital from January 2023 to December 2024.Based on the suturing technique utilized, patients were divided into the cosmetic suturing group(25 cases)and the debridement suturing group(25 cases).Clinical efficacy, perioperative indicators, pain levels, inflammatory cytokine levels, scar recov-ery,and the occurrence of complications were compared between the two groups. Results The overall clinical efficacy rate in the cosmetic suturing group was significantly higher than that of the debridement suturing group(P>0.05).Patients in the cosmetic suturing group exhibited a significantly shorter duration of edema and incision healing time,as well as narrower scar width compared to those in the debridement group, although treatment costs were notably higher(P<0.001).Postoperatively,visual analog scale(VAS)scores for both groups showed a decreas-ing trend(P<0.001);however,the VAS scores at 3 days and 7 days post-surgery were significantly lower in the cos-metic suturing group compared to the debridement group(P<0.001).At 3 days post-surgery,levels of inflammatory cytokines including tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),and interleukin-6(IL-6)were significantly reduced in both groups(P<0.001);fur-thermore,the TNF-α,IL-6,and IL-1 levels were signif-icantly lower in the cosmetic suturing group compared to the debridement group at the same time point(P<0.001).Three months post-surgery,scar assessment scores us-ing the Patient and Observer Scar Assessment Scale(POSAS)and the Vancouver Scar Scale(VSS)were significant-ly lower in the cosmetic suturing group compared to the debridement group(P<0.001).The overall complication rate in the cosmetic suturing group was significantly lower than that in the debridement group(P<0.05).Conclu-sion In the emergency management of patients with facial trauma, cosmetic suturing techniques are more advan-tageous than debridement suturing techniques in promoting wound healing, reducing scar width, alleviating postop-erative pain, and minimizing inflammatory responses, demonstrating significant clinical efficacy.
